High Risk Infant Program

The High-Risk Infant Program provides preventative and supportive services to families coping with high-risk pregnancies and premature or multiple births.  The program creates a support network for at-risk families by providing transportation, in-home support and referrals to other needed services such as counseling, evaluations from birth to age three, and medical specialists.  Additionally, the program's comprehensive services prevent high-risk families from becoming involved with the child welfare system.

UCAN provides the following in-home non-medical services:

  • Parenting support and guidance
  • Links to community resources
  • Review of the developmental stages of the
    child(ren)
  • Household organization
  • Meal planning and preparation
  • Assistance with medical care planning
  • Provision of a second pair of hands
  • Transportation and escort to medical appointments and hospitals
  • Specific, unique services as identified, such as those related to language or special development needs
